CMS & AMA Guidelines for Modifier 25: Coding, Billing and Reimbursement
06/22/2026 Live WebinarDescription
Modifier 25 remains one of the most frequently audited and misunderstood modifiers in medical billing. Improper use of this modifier can result in claim denials, reduced reimbursement, payer audits, and significant compliance risk. This webinar provides an in-depth review of CMS, AMA, and NCCI guidelines governing the appropriate use of Modifier 25 when an Evaluation and Management (E/M) service is billed on the same date as a procedure or other service.
Attendees will gain a clear understanding of how CMS defines a “significant and separately identifiable” E/M service and how that definition is applied through the Medicare Claims Processing Manual, CPT® guidance, and the NCCI Policy Manual. The session explores common payer interpretations, including Medicare and commercial insurance policies, and explains how global surgical periods impact reimbursement for minor procedures.
Through real-world examples, including NCCI policy scenarios, payer reimbursement policies, and “Yes or No” case studies, participants will learn how to determine when Modifier 25 is appropriate and when it should not be reported. The program also highlights documentation requirements necessary to support Modifier 25 usage, identifies services included in the global surgical package, and reviews recent policy changes affecting reimbursement.
This session equips healthcare professionals with practical tools to reduce denials, support compliant billing practices, and confidently defend claims when Modifier 25 is used appropriately.
